TREVISO. The health emergency linked to the pandemic has not only weighed on the patients of Covid 19: the commitment made in the care of infected patients has taken away energy from all other specialties. Without prejudice to emergencies, which have always been guaranteed, even a delicate field such as that of heart diseases has had to make sacrifices by slowing down ordinary interventions. There are hundreds of cardiac and vascular surgery operations on the waiting list. The head of Cardiology, and director of the neuro-cardio-vascular department of Ulss 2, Carlo Cernetti, has the recovery plan ready: the bulk of the operations that have remained dormant will be disposed of in the summer and we are almost there.

Waiting lists have also been formed in the field of care for heart patients, how will you manage them now?

«For cardiovascular activity where resuscitation is not needed, during the emergency we have increased the volumes of activity by 15-20% by helping other Ulss. It means that for example an aortic valve implant is not done with an open heart but going up through the artery of the leg, all in contact with the head of Cardiac Surgery Dr. Minniti. To manage the situation we have divided the department into Covid and non-Covid areas ».

Have you also operated on Covid patients?

“At the start of the pandemic, they looked like inoperable patients. Then we began to operate on them: by adjusting the valves we gave many the chance to heal, otherwise they would hardly have made it ».

And the branches that have slowed down instead?

“The other branches have been affected by the situation (because the Covid intensive therapies have absorbed the staff, ed): for a month or so they have restarted at full capacity. We plan to recover especially in the summer when the acuteness tends to subside. We will take advantage of it to dispose of cardiac surgery, neurosurgery and vascular surgery operations that require resuscitation. However, even in recent months, the chiefs have always checked the lists and if the patients became urgent they were operated on ».

Times?

“From 3 to 6 months to return to a list expected to be considered normal, but those who are getting worse will have a fast track”.

Are the operating theaters now there?

«Yes, on the basis of those in need, the rooms are modulated according to those who have greater needs. All the structures are able to guarantee an ordinary number of operations: from 3 to 5 interventions per day plus the emergencies for which a room is always kept free. As a timetable, if needed, we work on Saturdays or with an additional shift from 2 pm to 8 pm ».

And the staff?

“He did a truly commendable job: head nurse, nurses and all colleagues. Of course we are all very tired now and we hope to catch our breath. However, with the pandemic many doctors stopped in their specialties have helped in the Covid wards, now they all help to accelerate where we are back ».

How many interventions do you do per year?

«Between Treviso and Castelfranco 1600 heart angioplasty, 180 valve implants through the femoral artery with awake patient, 50 other heart interventions and about 200 non-heart angioplasty. Then a thousand pacemaker implants, in oenodynamics 200 angioplasty in Treviso in the acute phase of a heart attack and 150 in Castelfranco. We have a very low mortality. There is always a doctor available who at any hour in 30 minutes intervenes on the heart attack ».

The most critical moment?

“Since we have been overwhelmed by this misfortune we have always been motivated by the idea that there was an end. It was tough when we realized that the waves hadn’t stopped and the timing of the vaccines was unknown. The vaccine is saving lives, this restores confidence in the future and in the technology to fight tragedies that were taking away entire populations. We should be proud of our technoscientific level instead of getting sometimes lost in controversy over a queue or other trivial problems ». –
